Follow These Steps To Complete The Release of Liability Form

  • Download and print the Release of Liability form provided by LifeMission Youth. Download Form
  • Read the entire document carefully to understand the terms and requirements.
  • If the student is a minor (under age 18), ensure that both parents/legal guardians and the student are available to sign the Release of Liability form.
  • If the student is 18, they may complete this Release of Liability form notary process on their own.
  • Fill out all required sections of the Release of Liability form (student info, parent/guardian details, medical/emergency contact info requested).
Important: Do not sign the form yet—signatures must be completed in the presence of a notary.
  • Locate a notary public (banks, libraries, law offices, and UPS stores often offer notarization).
  • Bring the completed (but unsigned) form, valid ID for both parents/legal guardians and the student (if available), and any payment required for notarization.
  • All parties (both parents/legal guardians and the student) must be present.
  • Sign the Release of Liability form in front of the notary, who will verify identities and witness signatures.
  • The notary will complete their section, affix their seal, and provide a notarized copy.
  • Upload a scan or image of your completed and notarized form when you register.
  • Promptly turn in the original notarized copy to LifeMission Youth for camp records.
  • Turn in the original to an Offering Dropbox at your campus on Sunday or Youth Night.
Deadline: All hard copies must be turned in by June 24, 2025.
  • Schedule the notary appointment early to avoid delays.
  • Return the original paper copy to Sonya as soon as possible so it doesn’t get lost. If the original paper copy is lost or thrown away, the process will have to be fully done again.
