Follow These Steps To Complete The Camp Liability Waiver:

1. Obtain the Liability Waiver Form

  • Download and print the waiver form provided by LifeMission Youth.
  • Read the entire document carefully to understand the terms and requirements.

2. Gather Necessary Participants

  • If the student is a minor (under the age of 18), ensure that both parents/legal guardians and the student are available to sign the waiver. This is required to acknowledge consent and agreement.
  • If the student is 18, they are allowed to complete this waiver notary process on their own.

3. Complete the Form

  • Fill out all required sections of the waiver, including the student’s information, parent/guardian details, and any medical or emergency contact information requested.
  • Do not sign the form yet; notarization requires that signatures be completed in the presence of a notary.

4. Arrange for Notarization

  • Locate a notary public. Many banks, libraries, law offices, and UPS stores offer notarization services.
  • Bring the completed (but unsigned) form, valid identification (ie. Drivers License) for both parents/legal guardians and the student (if driver's permit or license is available), and any payment for the notary’s service.

5. Attend the Notarization Appointment

  • All parties (both parents/legal guardians and the student) must be present at the appointment.
  • Sign the waiver in front of the notary public, who will verify the identities and witness the signatures.
  • The notary will complete their section of the form, affix their seal, and provide a notarized copy.

6. Submit the Completed Waiver

  • Upload a scan or image of your completed waiver.
  • Promptly turn in the original notarized copy of your waiver to LifeMission Youth for their camp records.
  • The original notarized copy can be turned in to Sonya Houghton on Wednesday nights in Olathe, or
  • Put in a sealed envelope labeled ATTN: Sonya Houghton and dropped in an offering box on Sunday.
  • All uploads and hard copies must be turned in by June 30th, 2025. 

Tips for Success

  • Schedule the notary appointment as early as possible to avoid delays.
  • Return the original paper copy to Sonya as soon as possible so it doesn't get misplaced or lost.
